Good evening
I have been a type 1 diabetic for 14 years on 5 injections a day,2 long acting and 3 rapid acting.
I decided to find a new specialist as I had been struggling for a long time. He decided to check if my pancreas produced any insulin in which we discovered it does. So he has now changed me into metformin 1gram twice a day and a mixed insulin twice a day but my sugar levels are constantly running high any help would be greatly appreciated

I have the GAD test and showed no antibodies in the pancreas and now waiting or the c- peptide results

Hi. I think the c-peptide test result will be very important. Some T1s and quite a few LADA's will have some insulin production but still need Basal/Bolus and from what you say you still need it. My c-peptide was 1.19 within the range 1.1 to 4.4 but my DN and I found I needed Basal/Bolus and over the last 3 years my Basal has increased implying I've been in a honeymoon period. My GAD was negative. GAD results don't prove everything and if you have some but quite low insulin you need Basal/Bolus. I think your specialist may have jumped the gun?

If your levels are running high then I'd definitely go back and see if you need your insulin and meds regime changed.

Do you count carbs? How was your blood sugar on your old regime compared to your new one?
 
You can request a GAD test also as this should be a clear indication as to whether you are a type 1 diabetic or not.

It is not a clear indication, especially in a longtime diabetic. GAD is only one of four autoantibodies that should be tested for. There is sometimes the phenomenon that in the years after a diagnosis of diabetes, the antibody levels dwindle to the point that the tests are negative.

Properly, the patient should be able to ask the doctor to explain the diagnostic algorithm and the details of test results. If however the appointments are limited to several minutes, the patient is put in the position of asking Dr. Google.
