I don't have any idea why it might have happened, but I had background retinopathy at diagnosis in 2016 and a few weeks ago had a second test and the results were normal - so it is not a permanent state.

Ironically, fast A1c control can cause these changes? (Possibly the tightening of BP too.)

I can empathise it's not the sort of news you need after all the hard work on the management..
From our experience, the general upset to the eyes, caused by raised blood sugars does seem to carry on one way or another for quite a while after sugars (and, presumably insulin as the improvement is totally due to diet) are brought down to normal levels. However, James' eyes have, overall, improved enormously over the past five years of low carb, I recall the opthalmologist using the word, "amazing", at one point. However, there was one problem at the almost four year point, where a healthy eye got into an over exuberant healing mode, but this was dealt with by one VEGF injection. This shows how important it is to continue to have eye checks, even if diabetes itself has been resolved (other words are available!).
For eyes (and probably everything else), Don't panic, keep low carbing and carry on.

My next Hba1c two months later was 45, then in the Sept was 36, and has been in the 30s ever since. I just received this year’s results and I have “some background retinopathy”

Yup, moi aussi, didn't think the English would work....
First was clear. Second shocked the zap out of me when it came back "some background retinopathy”, phoned in to be told nothing to worry about!! Since then all clear... with my next test on Wednesday coming, so fingers are into crossed mode.
