Hi I ve been diabetic since i was 12 (type 1) Got my 3 month average done there last week and results were not good 12.0.
I exercise regularly and weigh about 81.5 kg. have a low carb diet and dont drink much only on weekends. last time i got
my average done was two years ago and it was 9.0. Can anyone give me some advice ??

Hi, quick question, how old are you? Second one is, how much insulin are you taking? The hba1c at 12 indicates that your blood sugar has been running at some very high levels over the past three months, and I would be surprised if you hadn't seen these in testing. How often do you test?

Sorry about all the questions, but for a t1, the immediate answer to why is my hba1c so high is that you haven't had the required control and your blood sugars have been too high.

With a 12, I'm surprised you haven't been recalled for stabilisation.

Have you done anything like the DAFNE or BdEC courses? These are also good for helping you stabilise your insulin and sugar levels.

Let us know the answers and we'll be able to help more effectively.

Every question asked here is key, get back to us @RB1 and we'll be able to help you from there. One thing @tim2000s didn't ask, that I would say is necessary... Why hadn't you checked your A1c in 2 years?

Hi Thanks i m 26 and i have a insulin pump. im on 1.3 units per hour. I ve Been on the pump since i was 15. I dont drink much alcohol and have a
low carb diet. I weigh 81.5 kg and go to the gym 2-3 times a week. I test myself i d say 6-8 times a day minimum. I think the main propblem is
that i drink too much lucozade when my sugars go low. other than that sugars seem balanced. I recently had my eye test and everyting was perfect. Im going to see my diabetes doctor in a few weeks so hopefully my results will be down a bit. thanks

Hi Thanks

Havent had it done in two years i know stupid of me i will have it done again in a few weeks when i go to see my diabetes
doctor

so was your HbA1c 12.0%?

This seems un-achievable going by the description of your control.............

I was at 12.0% right before DAFNE at the end of 2008, and I was not looking after myself at all, in fact I rarely took my insulin, which back then was Humulin M3, a 70/30 split.....

for it to be that high and for you to believe you have good control apart from when your low then you would think the problem area would be visibly obvious from blood sugar results....

how often do you go low?
how much lucozade do you consume when low?
what level does your blood sugar shoot up to after it?

have you carried out basal testing recently?

yes 12.0 was my HbA1c

I go low maybe twice a day and then drink 1 bottle of Lucozade and then my sugar goes to 16-17.
I think this is the problem i only need a small glass as oppose to drinking a full bottle

Over correcting a hypo is quite common in type 1's and this was a mistake I made for years, only by learning from experience can you determine how much glucose you actually need when having a hypo.

Hopefully your diabetes team will highlight where you are going wrong, good luck and do report back on how you get on.

If you're having a hypo twice a day, your basals may not be correct, Or perhaps your carb ratios are wrong. Is there a pattern to when you hypo?

Can I suggest changing what you do for hypos and using glucose tablets instead? That way it is obvious about the amount you are taking and you can't drink the whole bottle.

To have an HBA1c of 12%, if you are hypoing regularly, you would need to be spending a huge amount of time at a very high blood sugar level. 12% is the equivalent of an average level of 16.5mmol/l.

If you are hypoing twice a day (3 mmol/l lets say) and spent half the day at 8mmol/l, you would still need to be spending a large number of hours a day with a blood glucose of 25mmol/l  to get an average at 16.5.

When was the last time you basal tested?! And how often are you testing? Sorry if you have already stated this!

My hba1c used to always be around the 12-14 mark accompanied with frequent DKA....something needs changing! My hba1c is now about 7.5 I think....

Hi. It sounds like your insulin needs completely re-calibrating. You should not be going hypo even once a week let alone twice a day. Not having an HBa1C for 2 years is not good; for you it should be every 3 to 6 momths. Drinking a full bottle of Lucozade is, to be honest, madness. Do have glucose tablets instead unless the hypo is serious. These have around 8gm of glucose and you can take 2 or 3 or whatever when you start hypoing and that way you avoid over-shooting and then going high. I think you need to see the DN to discuss pump settings.

Hi thanks for the tip i ve reverting to glucose tablets instead of lucozade when i get a hypo. it seems to be working better, I m also recording my blood sugars every day and they seem to be stable thanks again

Hi thanks i would only drink a bottle of lucozade when my sugars are like 4.0 or under. The problem i also have i drink a bottle before go to the gym cause the training i do is quite intense. I dont think i need it maybe some glucose sweets instead. I ve been keeping a record of my blood sugars for the last week and there good usually between 5-7 in the morning and stable after meals like 6-8. i m going to see my diabetes specialist in a few weeks so hopefully i can get my result down to like 9-8 and then work on getting down from there.

Try having lucozade tablets on you have 3-4 of them followed by a protein bar this will stop the massive rises and if your going to the gym and your bloods are lower than 7 then have 3 of the tabs as this will stop you hypoing

Sorry.. 1 glucose tab pre gym will not stop a hypo....

Very much depends on what you are doing in the gym.

My experience is that if I am doing weights, then normally I don't need glucose tablets before the gym as the liver dump that takes place to fuel the muscles accounts for itself.

If I am doing aerobic cardio, it's a very different story.

Exactly.. a generalisation of 1 tab being sufficient is not exact for anybody to go to gym with.

Personally pre exercise I have a ramekin of berries, with cream and flax, chia seeds and that keeps me level for workouts as it is a longer term fix due to the fats...

Sorry 1 tab for my workouts would really be insufficient.

Thanks i found taking the sweats useful as oppose to lucozade. I hope to get my average down to 7-8 next time in a few weeks. What worries me is later on in life i.e 10 years that i might have eyesight problems? would this be the case if your average was 8-9?
