Survey on type 2 diabetes for a project

You make assumptions on the second detailed page about sharing data with health care professionals which are not valid and then don't allow a response to that effect. There is no "I don't share this because my health professionals never ask" response.
 
Done! Could have done with the choice ‘not applicable’ on a couple of the questions.
 
I started this but like resurgam couldn't get beyond a certain point It came up with error weblink message! I also wonder what you mean by healthy diet wonder if it's the same definition I'm working to ? For me it’s low carb and healthy fats. I no longer eat at least five portions of fruit and veg but do consider I have a healthy diet!

My shared data with GP has been automatic for yrs now. Soooo long I take it for granted that their pc has it all.

Key to improved services I think is to just have the diabetes educator pass a slip of paper to the patient with a link to this web site.
 
I also could not get passed a particular page, with message saying "please review information in other box" despite several attempts at reviewing it!

Each question needs a not applicable choice.
 
I got stuck at the box asking about what you find difficult about management of diabetes. Couldn't get past that page

I am not sure, though, that an answer that I don't track injected medication is helpful when I don't inject.
Also, recording BG readings; I sometimes use a computer application, my failing memory, a computer program such as Excel, paper and pen. So some questions I think should be multiple choice or say "which of these do you most commonly use".

Completed using Chrome on Windows 7 64 bit professional.

I see above that some people are having problems with completing the survey. This may be a browser issue because I had to go back a couple of times when I hadn't ticked on an option but I got a dialog box and a red comment by the box that needed filling.
 
Done. Like others have pointed out, some of the questions needed an n/a choice; there's no choice for an old-ish school Excel spreadsheet to track data; that question about tracking blood glucose as recommended by a healthcare provider/number of days is misleading - what if you track your glucose but haven't been advised to; healthy diet needs explaining more, as I think I do, but it might not conform to what most people would count as a healthy diet?
