Thankyou @buckmr2 .
Do you still find the aftercare telephone number any use?

You had your op around same time as my none diabetic friend. She has lost 11st too.
Im not expecting to lose that much and have been told and also no guarantee i will be able to stop the insulin.
However, as you know your body starts treating other meds differently too so i might be able to tolerate another type2 med again rather than solely rely on insulin.
I do have to wait and see.
The op is mainly for weight loss and anything more is a bonus.
I'm not even sure what insulin I'd be on after op if I do need it.
I have had a few insulin regimes now. 2 pregnancies. 1 with metformin too. I've had mixed insulin and now seperate basal and bolus again.

I bet insulin sensitivity is difficult as your stomach isn't as big as normal type1s. Have you been given half unit insulin pens?

I've not heard of half unit pens and can't see the point with so little difference between a unit and half a unit.
I've never used an aftercare tel. number as the diabetic side of my meds are overseen by my diabetologist.

Is there sign of your bgs improving?
How often do you see your dn?

So do you feel the diabetes has got worse since having your op?

I wondered why my specialist said I was severely insulin resistant.
My 83units of basal must be the clue?
Thanks @hawker101 .
Does your lantus reduce when exercising more?

I had bariatric Roux-en-Y bypass a couple of years ago due to worry over insulin intolerance after being on insulin over 40 years and the sheer amount of insulin I was on.Prior to the surgery my BG was well controlled
Post op (I lost around 12st in 6 months which was great) but a nightmare to get my BG back under control due to much higher insulin sensitivity.
Good luck with the op and subsequent control.
I've just been reminded.
All diabetic bariatric patients have to be under good control before op. Otherwise it can be cancelled.
Are you in any monthly meet ups to see if other diabetic patients are having same trouble? Are they having exactly the same problem?
I'm to join in soon so I'll be asking, I think I need too.
Thank you for making me aware @buckmr2 . It's people like you who have the experience to make more aware.
Would you not do it if you had to do it now, with the knowledge you have?

Have a look at "The Pioppi 21 day lifestyle plan" it is written by some superb medics and talks a lot about insulin resistance and links to other health probs.

I see tgeres no response to the question about weight. But circumference of weight is more an indication of health than BMI...

So you may find that you could reduce insulin if you can carry on low carbing or following Dr Aseem Maholtras Pioppi lifestyle.

At end of day, it doesnt matter how much insulin you take providing your bloods stay in target...

It would be easy for me to panic and think 'I'm on insulin now, just accept it'.
Another thing this forum has taught me is you don't have to settle for your current diabetic status.
Its ok to aim higher.

I'll be honest thou. When will I know I've done my uttermost?
I think I've just answered that on editting a post from 3wks ago. If there's room for improvement I should aim for it.
I'm excited about how far I can take this severe insulin resistance improvement.
I've read and listen to every post I've messaged on with regards to IR. Ultimately thou, experience has taught me the way to go, as I've done it before not low carbing. So with the added help of low carbing it should be possible to change this around. Right?

For me Dr Aseem research etc looks at whole of illneses and life.
Really interesting info in his book relating to carbohydrate intolerance.
Good explanations ref cholesterol and life and stress etc....
I think he is an all rounder health interest rather than just looking at one disease..
Which has led him to look at Italy and the folk who live to 90... and how etc..

We are all individual though...

Since I went on the Pump I am on 183 iu's a day of Novorapid, prior to that on Basal/Bolus I was on 120iu's twice daily of Lantus and 60 iu's per meal of Novorapid, also on 2000mg of Metformin ER and 15mg of pioglitazone (HB1AC was running at 117% at the time) I am still massively insulin resistant, even when I lost a lot of weight and was exercising 5x a week. Seems like it is my lot in life at the moment for my body to not want to recognise any insulin that I put in, on a positive note I have got my HB1AC down to 50%, morning BS is 5-6mmol and post meals are usually the same. So am putting the work in, average grams of carbs are 40-70 a day depending on what veggies I am eating, but all in all am just plodding along

I am taking NOVORAPID 38 UNITS morning, lunch and evening time

Then my Lantus 100 units at night time

I feel this is quite a lot but sometimes my sugar is still high even I am also in low carb diet

I am type 2 with metformin SR also 500 mg 2 x a day (suppose to be 1000 but stomach upset is getting bad so I have to adjust for a while)

What insulin you are having??
Interesting seeing all the different dosages - I always thought my insulin consumption was Higher than most people - but based on this thread, I guess not. The fact that I'm Type 1, a good body weight and very physically active - I guess all plays in to this but here's what I take on an average day:
Non fasting day - Humalog (12 units morning, 10 units lunch, 12 units supper) = 34 total units of Humalog
and for Lantus - 23 units before bed time
Semi Fasting (weekends) - Humalog (5 units morning, 10 units supper) = 15 Total Units
and for Lantus - 23 units before bed time (A total fast means less insulin but I still prefer to have one meal for the day so I call it semi fast)

So on a normal non fasting day my total accumulation of both insulins - 57 IUs
Semi fasting - 37 IUs

I've noticed with continuing weight loss I have been able to reduce my toujeo300 to 66units from 69 and novarapid to 45units from 60. Giving me fbg between 4.5-6.9 depending on how late I eat.
Seeing dn next once instructed to start liver shrinking diet for bariatric surgery. Most have to half their insulin need.
I'll be interested in what change I have to make. Very interesting in what need; suits.
I'm wondering if basal can be lowered more as my weight continues to drop.

I'm on 14 units of lantus at the moment. For bolus I was having a total of probably 60 units per day on average. At the moment I've dropped the dosage to about 30-40 units because I've been replacing some meals with a glass of semi-skimmed milk. I seem to tolerate the milk without needing insulin. The only reason I'm drinking milk is that I'm on meds that require food or milk to take them otherwise I'd be fasting some meals. This is my work around to calorie reduction with my weird diet thanks to all my illnesses. So far so good and the weight is starting to move in the right direction. I just got to my wits end with the med dosage changes adding more weight every single time... so this is my way of fixing it. I'm aiming to keep calories at 1500 or thereabouts at the moment for a while until my stomach is feeling more comfortable. I have so much pain in my stomach as it is and the last thing I need is more weight going on it. Wish me luck though as tomorrow I have a lesson to learn to self catheterise myself and I'll have to do that 4 times per day.... just another thing to add to my "to do" list.

Fantastic work @Mep .
I'm on 63 units of toujeo300 basal and 25-40units of bolus daily now.
Had a near hypo an hour ago which is similiar to yesterday too.So reduced basal from 66 to 63 this morning. Might have to reduce again in a few days. I hope!
I love these reducing insulin and weight days. :) :) :)
Im sure you will rock this like all other duties you have to do to keep healthy.

I'm on 18 units Lantus daily which seems to keep me stable at around 6 provided I only eat around 8 carbs per meal. I also take 1 X Forxiga daily. Guess I'm lucky compared with a lot of others on this site. I do find that a good walk (at least 5 miles daily) can reduce my Lantus to 14 or 16.

I'm pretty rigorous with the carbs as they can sneak in where you least expect them, wicked blighters!

Currently at 13-14U Lantus in the morning, then 3-6U Apidra each meal depending on BG. I may add another 2-4U Apidra if i get snacks, but thats pretty rare.
So i average on 13U Lantus and about 11U Apidra daily.

I got my basal down to 54 units then Xmas cars struck. Argh!!!!ha ha
So on 60units toujeo300 an 60-80units of novarapid. Will reduce basal again when fbg more settled as huge lergy at mo (I think laryngitis).

Ideally I'd like less than 50basal and 60novarapid.
Onwards and upwards.
Off metformin again as just over 24hr tummy bug. So with metformin on my current insulin I shouldn't be far off. Fingers crossed.

48 units of Novomix 30 in the morning - night time can vary but on average it’s 38 units of Novomix 30.
