NHS Dietitian Contradicting Himself

This sounds strange. I was sent a letter calling for my HbA1c blood test from the surgery and up until now have had the bloods drawn at the surgery. This time the letter states I have to go up to the hospital or to another surgery just to have the bloods drawn. I am unhappy about this and I am about a month overdue for the tests as I do not see why a simple blood draw has to be done elsewhere. I had deliberately changed GP/Surgery so as to cut the inconvenience and cost of going to my old surgery, now it will cost twice as much.
You'll probably find that the phlebotomist has left so they can't/won't do blood draws at the surgery? If the phlebotomist isn't there then none of the others at my place will do mine. She's the only one that can get blood out of me!

You'll probably find that the phlebotomist has left so they can't/won't do blood draws at the surgery? If the phlebotomist isn't there then none of the others at my place will do mine. She's the only one that can get blood out of me!

The very first draw was done by the DN herself. She may have left, I suppose, but my surgery is supposed to be a health centre and offers a multitude of services. At my old surgery a phlebotomist visits one per week and that surgery is not a health centre. All this cafuffle is putting me off having the test done.

NHS - in instruction video for usage of CGM for T1D states, that fats are changing to glucose, but later than carbs, is it true? Is there same simple description of this process? Is T1D metabolism other than T2D according to fats?

No - fats do not become glucose when digested - that is why it does not cause insulin to be released.

So why the NHS says in instruction to bolus for the fats? And there is also the math for calculation from calories. From 1:45:20 - I know, it is in czech, but the math can be seen on presentation:

If I were to bolus for fat, I’d hypo. I inject for carbs and protein only. Fat doesn’t turn to glucose :)

You cannot convert fats directly into muscle-ready glycogen., though through a series of metabolic processes that result from conditions of depleted carbohydrates, it is possible for stored fats to be broken down into glucose, which can then be converted into glycogen.

So there is no direct way that ingested fats can affect blood sugars.

Just had a visit with my NHS Dietitian.

Whilst congratulating me for going into "remission" and lowering my HBA1c from 85 to 38 in just over 4 months, he did not like the fact that I changed my diet to an extremely low-carb and very healthy diet.

He said my fasting blood glucose readings between 4 - 6 mmols/l are false and I'm cheating.

He also emphasized to me to eat "normally" and stressed out that the NHS does NOT agree with low-carbing and it's strongly NOT advisable.

So I was sitting there in his office thinking this is just another waste of my time and while I was listening to his sermon, I saw a book sitting on his desk about "Carb and Calories", the best selling book on counting carbohydrates and calories for Diabetics!!!

Absolutely confusing!!!!!!!

I had a similar experience with my doctor when I dramatically lowered my readings. I went to see her so she could adjust my medication, instead she suggested I eat MORE carbs! I have since changed doctors and no longer see the Diabetes Educator who had the same attitude as my original doctor.

I know this is going to sound awful but the when I saw "Health Care Assistant " at my local surgery and was being asked if I was eating a healthy diet and taking regular exercise all I could do was look around the room for the hidden cameras, I was sure I was on Candid Camera or someone was playing a practical joke. This women that was asking me all these questions about diet and exercise was almost as wide as she was tall. Not exactly setting a very good example.
How sad you felt the need to be unkind about her weight. There are lots of reasons why she may be struggling. I have been battling for 55years due to hypothyroidism. Just one of many causes for someone to gain weight. In all that time only one GP has given me the dose I need to feel well and loose weight. I am type 2.

doctors and dentists.I would not be rude to any.Also i don't like being taken for granted.
Hello .For years i've had blood taken at the surgery by the practice nurse .Then i received a letter saying for future blood letting.,i had to go to the surgery for a blood form .Then go to either Asda supermarket or the gerneral hospital for the blood to be taken.I am also unhappy about this.They seem to be doing less and less.Seeing i have severe osteoarthritis in my left hip .It makes even a trip to answer my door very painful.They could fit me with a sparkling brand new hip.but they will not hear of it.so i will have to grin and bear it.

In the meantime, if you can control your blood sugar through diet and exercise, this is preferred to meds. Meds have side effects.

Be sure your metabolism doesn't suffer too much by going too low carb too fast. Carbs and gylcemic index are directly related. If you go low GI, you are also going low carb.

Lose 1 to 2 lbs per week if you need to lose weight. Not any faster, or, you could bring on a heart attack... congestive heart failure, shoot up your cortisol, which steals from progesterone which in women keeps estrogen in check..., thereby throwing your hormones out of whack, which can lead to bad things like cancer from too much of a certain type of hormone. Among other things.

Unfortunately I have had quite a few spells in hospital since my sixties, both for myself and my wife. I am dismayed when I see so many NHS staff who are clearly obese, among them people who tell me my BMI (Which I think is a load of Bovine Manure.) is too high. It makes we wonder whether or not the NHS will ever admit to the truth on diet; because I am sure they do know the truth. I detest this 'eat healthily' chant, and this nutritionist who is accusing someone of falsifying their readings should be struck off. He is obviously a brainwashed fool, who blindly accepts conventional wisdom, without asking why the 'patient' is claiming these readings. The fool needs re-educating, but sadly that won't happen will it? The establishment either doesn't have a clue, or there is a disgusting conspiracy going on. I suspect the latter, with the younger doctors and consultants in ignorance of the truth about diet. All for the benefit of 'Big-Food' and 'Big-Pharma'.

I have one answer for them when they advise me to 'eat healthily'. My reply is, 'I do eat healthily and that's why I have shed almost four stones since Sept/October 2017.' I feel fine and I don't care what these people tell me. (As long as they don't insult me.} When I shop for food I buy only what was once running around, flying around or swimming in the seas. As for Carbs I stay with greens, that can be picked from a bush or a tree, and the only fruit I eat are berries of whatever species I can get. I don't drink just water, of course, although I suspect I should do that. That's all that wild animals get, and they seem to manage. But I don't think I will give up my one cup of coffee per day!

My blood sugar readings are now rarely above 7.5, and after conducting my own fasting reading, my readings regain normality after about 1 hour. I have never felt better since my forties. I still have some way to go (I am 17 stones 10lbs) but I am getting there. Instead of castigating me for not eating whole-grain bread, and brown rice, I think my Doctor should ask me how I did it. After all he knows my readings as well as me. But has he remarked on it? Not on your life. My one regret is I can't stomach the taste and texture of avocado pears. It must be like eating tallow candles!

Tell your nutritionist to observe wild creatures.They eat only what is available to them, and they all specialise. We used to be wild, long before bread and rice, and we ate what was available to us. Namely our natural foods that we could gather or hunt. We are still the same, and whilst we don't have to gather and hunt, those foods are still available.
A simple philosophy I suppose, but now I have accepted it, it's working for me too. My lowest reading was 5.8. And I ain't cheating!!!

yesterday the Diabetic Educator said if she didn't know I was 70 she'd think the blood results were that of a 19 to 20 year old
If you haven't already, I strongly suggest posting a version of this on the "Success Stories & Testimonials" Forum where people needing encouragement can find it.

Namely our natural foods that we could gather or hunt. We are still the same, and whilst we don't have to gather and hunt, those foods are still available.

Reading this, I reflected that living as I do in a big city in the North of England, I'd have trouble feeding myself as a hunter gatherer. But then I realised that we do have an unlimited supply of fine, healthy looking rats. I'd need to be pretty adroit to catch one, though. Grey squirrels would be another possible prey, but it would take a fair few to make a meal.

Reading this, I reflected that living as I do in a big city in the North of England, I'd have trouble feeding myself as a hunter gatherer.

Why? Fresh meat, fresh fish, fresh vegetables and dairy foods are available everywhere I've looked. ;)

One of mine brought a live chicken in through the window last year, he’d pinched it from one of the nearby allotments. My other cat brought home a cooked, breaded chicken leg...

When hubby and I were abroad on holiday, and daughter was home alone (with a friend) we got a panic stricken phone call from her, which was direct to the complex as no mobiles in those days, so the manager had to come looking for us. Our cat had dragged a dead, mangled rabbit in through her downstairs bedroom window and plonked it on the bed. Neither she nor her friend dare go anywhere near it. I think she expected us to get the next flight home. :hilarious: We didn't.

In West Africa, however, rats are a major item of diet. the giant rat (Cricetomys), the cane rat (Thryonomys), the common house mouse, and other species of rats and mice are all eaten. According to a United Nations Food and Agricultural Organization report, they now comprise of over 50 percent of the locally produced meat eaten in some parts of Ghana. Between December 1968 and June 1970, 258,206 pounds of cane-rat meat alone were sold in one market in Accra! This is a local recipe that shows the South American influence on West African cuisine.

Stewed Cane Rat
Skin and eviscerate the rat and split it lengthwise. Fry until brown in a mixture of butter and peanut oil. Cover with water, add tomatoes or tomato purée, hot red peppers, and salt. Simmer the rat until tender and serve with rice.

Your cat is smarter than you realize.

In fact with cauliflower rice it would be quite a low carb meal.
