I am posting on behalf of my son who is 9 he has a condition of ketotic Hypoglycemia. He typically has lows when fasting or ill. However, over the last few months he has lows after eating. He has just come out of hospital where he completed a 20 hour fast white lowest sugar being 3.2. He then has a gtt test today and I have uploaded the initial results. Can anyone provide any advice on them so far?

Insulin level after 2 hours 1321
C peptide level 5495
Glucose to insulin ratio 0.50

Following the fast he ate his breakfast and then had a low of 2.4. We also use dexcom

Hi @Kingylow

You've posted quite late in the UK day for the majority of the UK posters, so you may have to be patient to get input from them or from the overseas folk such as myself.

I must admit, I hadn't heard of it before today, though it does get an occasional mention in these forums

I'll tag @Lamont D who knows quite a bit about reactive hypoglycemia.

All I can tell about it is that usually kids grow out of it eventually? (I know, not much help in your current situation, though I guess it's a plus that you have a definite diagnosis.)

Hoping your son gets help and/or grows out of this condition. Lots of virtual hugs from New Zealand.

I hadn't heard of it before either. Scary. Hopefully I read that children usually grow out of it like @EllieM said. You probably want to make sure it's not Reactive Hypoglycemia for the after meal drops. I don't know anything about ketotic hypoglycemia to know if dropping after meals can be related to that condition. But the dropping after meals is what's common in Reactive Hypoglycemia.

Thanks so much for your replies. It’s a condition that he’s had since he was 1 but the lows after eating are a new thing that has happened over the last six months or so. From research I’ve noticed his c peptide is really high meaning he is producing to much insulin?

Hi, yes I've have read about ketotic hypoglycaemia.
It can be described as hypoglycaemia that is common in children, when they can't eat or don't eat for a lengthy period.
there will be a time when the child should grow out of it, but if you do Google it, there is no actual treatment other than ensuring the child does eat regularly to forestall the hypos. I know all about producing too much insulin but mine is because of food intolerance mainly carbohydrates. You could try the child on less carbs as a trial but some paediatric specialists will have more experience of this type of hypoglycaemia.
hope you get sorted. Having hypos every day is not pleasant, indeed it's awful!

Another person who had not heard of it. That sounds really tough for both your son and you. More virtual hugs from me.
