Hi and welcome,

My advice is to use your new meter to guide you on your diet, which sounds like it needs tweaking. Test immediately before you eat and 2 hours after first bite. Keep a food diary including portion sizes and record your levels alongside. If the 2 hour rise is more than 2mmol/l there are far too many carbs in that meal. It is preferable to keep any rise under 1.5mmol/l. You can adjust the carbs by cutting portion sizes or by eliminating certain ones. Bread, pasta, rice, cereals, potatoes and flour are the biggest culprits.
Happily, fats do not raise blood sugar levels. In fact they actually help keep the rises a bit lower. You can safely ditch all your low fat products and eat the real thing.

The role of carbs is very important.

I will tag @daisy1 who has some excellent information for newcomers. Meanwhile, have a good read round these forum threads and ask as many questions as you like.

Hello Max and welcome to the forum :) Here is the information we give to new members and I hope you will find it useful. It contains info on carbs and a link to the Low Carb Program which you could join. Ask as many questions as you want to and someone will be able to help.

BASIC INFORMATION FOR NEWLY DIAGNOSED DIABETICS

Diabetes is the general term to describe people who have blood that is sweeter than normal. A number of different types of diabetes exist.

A diagnosis of diabetes tends to be a big shock for most of us. It’s far from the end of the world though and on this forum you’ll find over 150,000 people who are demonstrating this.

On the forum we have found that with the number of new people being diagnosed with diabetes each day, sometimes the NHS is not being able to give all the advice it would perhaps like to deliver - particularly with regards to people with type 2 diabetes.

The role of carbohydrate

Carbohydrates are a factor in diabetes because they ultimately break down into sugar (glucose) within our blood. We then need enough insulin to either convert the blood sugar into energy for our body, or to store the blood sugar as body fat.

If the amount of carbohydrate we take in is more than our body’s own (or injected) insulin can cope with, then our blood sugar will rise.

The bad news

Research indicates that raised blood sugar levels over a period of years can lead to organ damage, commonly referred to as diabetic complications.

The good news

People on the forum here have shown that there is plenty of opportunity to keep blood sugar levels from going too high. It’s a daily task but it’s within our reach and it’s well worth the effort.

Controlling your carbs

The info below is primarily aimed at people with type 2 diabetes, however, it may also be of benefit for other types of diabetes as well.
There are two approaches to controlling your carbs:

  • Reduce your carbohydrate intake
  • Choose ‘better’ carbohydrates

Reduce your carbohydrates

A large number of people on this forum have chosen to reduce the amount of carbohydrates they eat as they have found this to be an effective way of improving (lowering) their blood sugar levels.

The carbohydrates which tend to have the most pronounced effect on blood sugar levels tend to be starchy carbohydrates such as rice, pasta, bread, potatoes and similar root vegetables, flour based products (pastry, cakes, biscuits, battered food etc) and certain fruits.

Choosing better carbohydrates

Another option is to replace ‘white carbohydrates’ (such as white bread, white rice, white flour etc) with whole grain varieties. The idea behind having whole grain varieties is that the carbohydrates get broken down slower than the white varieties –and these are said to have a lower glycaemic index.
http://www.diabetes.co.uk/food/diabetes-and-whole-grains.html

The low glycaemic index diet is often favoured by healthcare professionals but some people with diabetes find that low GI does not help their blood sugar enough and may wish to cut out these foods altogether.

Eating what works for you

Different people respond differently to different types of food. What works for one person may not work so well for another. The best way to see which foods are working for you is to test your blood sugar with a glucose meter.

To be able to see what effect a particular type of food or meal has on your blood sugar is to do a test before the meal and then test after the meal. A test 2 hours after the meal gives a good idea of how your body has reacted to the meal.

The blood sugar ranges recommended by NICE are as follows:

Blood glucose ranges for type 2 diabetes
  • Before meals: 4 to 7 mmol/l
  • 2 hours after meals: under 8.5 mmol/l
Blood glucose ranges for type 1 diabetes (adults)
  • Before meals: 4 to 7 mmol/l
  • 2 hours after meals: under 9 mmol/l
Blood glucose ranges for type 1 diabetes (children)
  • Before meals: 4 to 8 mmol/l
  • 2 hours after meals: under 10 mmol/l
However, those that are able to, may wish to keep blood sugar levels below the NICE after meal targets.

Access to blood glucose test strips

The NICE guidelines suggest that people newly diagnosed with type 2 diabetes should be offered:

  • structured education to every person and/or their carer at and around the time of diagnosis, with annual reinforcement and review
  • self-monitoring of plasma glucose to a person newly diagnosed with type 2 diabetes only as an integral part of his or her self-management education

Therefore both structured education and self-monitoring of blood glucose should be offered to people with type 2 diabetes. Read more on getting access to bloodglucose testing supplies.

That was a chicken breast

If my daughters levels were this high I would be scared stiff

Hi. Yes, your blood sugar is high and it sounds like T1 is a possibility. Be aware that a negative GAD doesn't prove no T1 - a c-peptide test is then necessary to check your insulin level. Don't worry about butter or cheese or fats in general but do worry about carbs which I think you are trying to keep down. Are you on any meds yet? It looks like you should be on the typical set of three tablets already with insulin possibly needed.

No I'm not on meds yet ....they are going to wait I think they don't seem as concerned as me lol
But thankyou so much about the a c peptide test I'm going to have to google that because I haven't got a clue about that test
Cheers

If the GAD test is negative, you could be positive for ZnT8 autoantibody. They should also test for IA-2A and IAA because some people have 2 or 3 autoantibodies.

A very low value of C-peptide is a piece of evidence that one has T1, not T2.

Take a look at this Website's forum, Success Stories and Testimonials. Many members have achieved rapid weight loss and brought A1c value down by adopting low carb high fat. You have been avoiding butter and cheese. Read up on LCHF and try eating a lot of butter and cheese. It would probably help to avoid potatoes for a few weeks (very high carb).

I would suggest skipping the grapes and any sweet fruits for now. Consider lowering your carbs and see if your glucose levels stabilize. It is possible to have a good meal that has no more than 2 mmol increase in glucose level.

Would be good if your blood test includes fasting insulin level. Then you will have a fairly good idea on whether you are type 1 or 2. If you have high fasting insulin level, it would be much easier to manage the condition thru dietary changes.

All the best.

Would be good if your blood test includes fasting insulin level.

Yes, it seems to me the fasting plasma insulin would usually be more informative than the C-peptide. It's so important, I wonder why taking its measure is not routine. Knowing the fasting C-peptide is good too.

Most likely, the type of diabetes here is between LADA, T1, and T2. LADA and T1 are autoimmune diseases. It's possible to have LADA with features of T2, or T1 with features of T2.

Make sure your doctors know about the full range of beta cell autoantibodies. The following page is written for the layperson: http://www.bristol.ac.uk/clinical-sciences/research/diabetes/research/autoantibodies/

Well guys I went to GP this morning only to be told results of the GAD test not in
I took a food diary and levels ( which were not asked for )
Anyway they told me to stop monitoring my levels
Because it was giving me anxiety .(no ****)
Told me to come back in two weeks time to see if I'm type 1 with the GAD test
I've got high blood sugars 15 before food 20 to 28 2 hrs after

My diet is excellent ...and now I've cut out fruit ...levels are still high
Don't eat bread or pasta or fats or sugar

She told me she will try and see if she can send me to the diabetic nurse but she will let me know in two weeks

Came away so disappointed
OH FORGOT TO SAY DAUGHTER IS TYPE 1 has been since she was 11 she's now 23
So I'm not a novice lol

Sounded like Type 1 to me when I read through your posts here.

At least you finally have an answer. Having been a victim of faffing about from doctors myself, I sympathise. Hopefully, you can now move forward and feel better :)
