Ageing is the biggest known risk factor for dementia, but the reason is still not fully understood.

A new NIH-funded study points to one possible mechanism: the brain’s immune environment may change sharply in midlife.

Researchers studied post-mortem hippocampal tissue from 40 neurologically healthy adults aged 20 to 95.

The hippocampus is central to memory and learning, and it is one of the brain regions most relevant to Alzheimer’s disease.

The team used advanced single-cell techniques to look at the identity and behaviour of cells in exceptional detail.

They found that microglia, the brain’s main resident immune cells, gradually decline from around age 50 to 75.

At the same time, the brain appears to gain cells with stronger inflammatory signals and features more like immune cells from the blood.

That is a big deal because microglia have long been thought of as cells that develop early in life and renew themselves within the brain.

This study suggests the ageing hippocampus may be more dynamic, and potentially more vulnerable, than that model allowed.

The researchers also found signs of age-related decline in cells involved in maintaining the blood-brain barrier.

That barrier helps control what passes from the bloodstream into the brain.

If it weakens, inflammation and vascular damage may become more likely.

The study does not prove that this immune shift causes Alzheimer’s or dementia.

But it gives researchers a more precise place to look.

The ageing brain may not simply wear down.

It may also change its immune identity in ways that make chronic inflammation more likely.

For people with diabetes, the finding fits a wider pattern.

Metabolic disease, vascular health, inflammation and cognitive decline are tightly connected.

Protecting the brain means thinking beyond memory games.

Blood pressure, glucose, sleep, movement and inflammation all matter.

If future work confirms this immune shift is involved in dementia risk, it could open up new ways to protect the brain before symptoms begin.
